Where is my application?
  • If you are unable to access your application, please check to see if you are signed into SurveyMonkey Apply as an “individual.” SurveyMonkey Apply does not accept applications from “individuals” and only recognizes “organizations.” Look in the upper left portion of the website, and toggle to your “organization” before trying to access the application.
 How do I reset my password?
  • On the login page, please select “Forgot your password?” located in the bottom right corner. Enter your email, and follow the email’s instructions to reset your password.
Do I need to set up a username if I have applied before?
  • You will need to set up a username if you have not applied to a Seattle Foundation grant via SurveyMonkey Apply before.
